Virtual care is not appropriate for medical emergencies, chest pain, severe shortness of breath, stroke symptoms, uncontrolled bleeding, or other life-threatening conditions. If you are experiencing an emergency, call 911 or seek immediate in-person medical care.

Nature of Telehealth: I understand that telehealth involves the use of electronic communications to enable a healthcare provider at a different location to share individual patient medical info for the purpose of improving patient care.

Benefits & Limitations: I understand that while telehealth allows for convenient access to care, it is not a substitute for an in-person physical exam when one is clinically indicated. Certain conditions may require an in-person follow-up or referral to a specialist or emergency department.
